Transition Support That Extends Beyond Discharge
Case managers in UVA Health’s Population Health Department provide critical support to patients after discharge from the Medical Center. Assigned to patients upon discharge from the ED, ambulatory clinic, inpatient stay, or for continuity of care, our case managers work closely with state, regional, and local partners to connect individuals to essential community resources.
A smooth recovery begins with education. Our team ensures patients understand their diagnosis, medications, and care plan, and are connected to a primary care provider (PCP) and follow-up services. By addressing care gaps, reinforcing support systems, and promoting self-management, we help reduce avoidable emergency visits and readmissions—ultimately improving long-term health outcomes and quality of life.
