PATH

Pediatric Access and Team-based Health (PATH) is a care coordination initiative built to support children and their families through easier access to health services and local support networks.

With a focus on collaborative care, PATH simplifies healthcare navigation—especially during key transitions—and works to proactively address non-medical factors that influence a child’s overall health.

PATH's Mission is to provide coordinated, family-centered care for pediatric patients by improving access, building teamwork across providers, and addressing both health and everyday needs that influence long-term outcomes. Pediatric Population Health Case Managers work closely with clinics, primary care teams, Fortify Children’s Health, and local partners to keep care on track. PATH uses Epic Compass Rose to coordinate tasks and track patients in real time. Coordination starts in the clinic and continues through transitions to ensure continuity of care.
