Population Health programs are designed to support community members and patients and to ensure they have the information and medical support needed to improve and maintain their health. Several of our patient programs have a dedicated clinical team tasked with supporting this effort. Our Population Health clinicians and case managers are further supported by key ancillary team members.
The Ancillary Team is comprised of UVA Health Pharmacists and Physician Champions who contribute the following services:
- Medication reconciliation for adherence and access to medication assessment, education, and identification and resolution of drug-related issues
- Dedicated pharmacy resource to call IHM patients transitioning to home post discharge with a target of less than 48 hours
- Assist in locating pharmacies
- Provide medical oversight for Population Health programs and clinicians
- Assist in the development of new initiatives and patient programs
