Tyler Lienhart is certified as a Qualified Mental Health Professional (QMHP) through the National Board for Certified Counselors and holds both a bachelor’s and master’s degree in psychology. He joined the Transitions of Care team in 2026 as a Post-Acute Care Liaison.

Tyler brings a strong background in mental health and case management, with experience in cognitive behavioral therapy, person-centered therapy, and rehabilitation care. He is known for his collaborative approach, analytical thinking, and dedication to delivering high-quality, patient-centered care. He strives to support patients in achieving their treatment goals through practical, thoughtful, and individualized care strategies.

Throughout his career, Tyler has been committed to advocating for and supporting patients, parents, and families as they navigate mental health services and care coordination processes. His professional and academic experiences reflect a consistent focus on growth, learning, and excellence in service to the community.

Outside of his professional role, Tyler enjoys playing soccer, gaming, spending time with family, and being outdoors. He is motivated by helping others, fostering innovation in care, and continuing his personal and professional development. He looks forward to expanding his knowledge and skill set to better serve diverse patient populations and contribute to meaningful, outcomes-driven care.
