Hannah Lesniak started her nursing career in 2023 after graduating from James Madison University's BSN program. She has experience as an emergency department nurse, endoscopy nurse, and home health nurse. Hannah joined the Virginia at Home team in 2025 as an RN clinician to provide quality primary nursing care in the comfort of patients' homes.
Hannah is very passionate about patient education and minimizing healthcare gaps in rural settings. She believes that connecting with patients in the home greatly improves quality of care, addresses social drivers of health more fully, and results in positive patient outcomes. Outside of work, Hannah enjoys hiking, reading, traveling, and volunteering with various local organizations.
