Carly Frazier joined Population Health and the Virginia at Home team as RN Care Coordinator in December of 2025. She has been a part of the UVA family since graduating from Radford University with her BSN in 2015. For the first ten years of her career, Carly cared for adult general medicine patients, where she discovered her passion for building meaningful relationships and supporting individuals through every step of their health journey.
In her role with the Virginia at Home team, Carly is dedicated to helping patients navigate challenges, maintain wellness, and feel supported from the comfort of their own homes.
Outside of work, Carly loves spending time with her husband, daughter, and boxer puppy, George. She is happiest when she’s planting flowers, hunting for great thrift finds, or just enjoying time with her family.
