Nomination Form - HUC Customer Service Award

Any UVA Health System employee may nominate candidates for this award.  Nominees must be employed in a job with title ‘Health Unit Coordinator' for at least one year.

The following criteria are used in making the nomination.  The name of the nominee, unit, and a written description of how the nominee meets the criteria.  

Nominees must demonstrate the following qualities:

  • CUSTOMER SERVICE: Goes above and beyond to meet team members' needs in support of patient care.  Demonstrates customer service principles in all communications.
  • PROFESSIONALISM: Demonstrates positive, can-do energy in performing job duties.  Role models prompt, courteous responsiveness to patient and family needs.
  • PROBLEM SOLVING: Negotiates internal systems to meet needs in a creative manner.
  • TEAMWORK/COLLABORATION: Promotes team harmony to maximize quality service.

Nominations must be submitted by 5pm on July 15, 2014.
