Occupational Health Screening Instructions

If You Need a TB Screening Only

1.  If you have a history of positive ppd, complete the linked form and fax to UVA-WorkMed -  Positive PPD Form
2.  If you need a TST placed, you can drop by Employee Health (1222 JPA) during their work hours or call UVA-WorkMed (1910 Arlington Blvd) for an appointment.  WorkMed's phone number is 434-243-0075.

If You Need a Respiratory Fit Test and TB Screening

*NOTE:  If you have a full beard or mustache, a disposable respirator may not provide an effective seal.  Contact Jon Schuch (jzs@virginia.edu or 434-982-3845) to discuss options.  Otherwise, proceed as described below.

1a.  If you prefer to have the screening at UVA-WorkMed (1910 Arlington Blvd), call 434-243-0075 for an appointment, pressing 1 to be routed directly to the scheduler.  Explain to the scheduler that you are a Credentialed Staff Member and need a TB and RFT screening.

  • Do not eat, drink or smoke 30 minutes prior to your appointment.
  • Print, complete, and bring the linked health questionnaire to your appointment Health Questionnaire  

1b.  If you prefer to have the screening at Employee Health (1222 JPA), please follow the instructions directly below.

 

  • Log in to NetLearning:  https://lms.healthsystem.virginia.edu
  • One you are logged in, highlight the "Learning Opportunities (Enroll)" button, and enter "annual respiratory" in the search box.
  • Scroll the list of classes to find the class you would like to attend.
  • To sign up for a class, click on the blue "Enroll" button associated with the class.
  • Do not eat, drink or smoke 30 minutes prior to your appointment.
  • Print, complete, and bring the linked health questionnaire to your appointment Health Questionnaire
  • If indicated, a TST will be placed during the RFT session.

If You Need Hepatitis B Screening

Hepatitis B screening requires an antibody test and, if the antibody test is negative, an antigen test to rule out active disease.  Hepatitis B screening is by appointment through Employee Health (1222 JPA, 434-924-2013) or UVA-WorkMed (1910 Arlington Blvd, 434-243-0075).

If Hepatitis B vaccination is needed and desired, UVA-WorkMed can provide vaccination at no cost.

If You Need to Address Mumps, Measles, Rubella, Tetanus Booster, or Varicella (Chicken Pox)

Vaccination is available through UVA-WorkMed at no cost.  Call WorkMed for an appointment, if you desire vaccination (434-243-0075).

You can also decline vaccination or attest to a physician-diagnosed case of mumps.  Links to these forms are directly below.

Regarding Varicella, if you are unsure whether you have had the disease, a titer is indicated and is available through UVA-WorkMed at no cost.
