FY17 Self-Appraisals

Self-Appraisals are designed to allow an individual to evaluate themselves on their fiscal year (FY) performance.  The online form is structured in a similar manner as the official Performance Appraisal.  A status of "Evaluation in Progress" indicates the Self-Appraisal is open/available.
Self-appraisals are required for any job role where goals are entered and approved within ePerformance.  For FY17, these include leadership roles as well as the RN Pilot Group (Clinician 1 - 4 & Wage RN job titles). 
In addition, some managers will require team members in other roles to complete a Self-Appraisal.  If you are impacted by this requirement, your manager will notify you.
