What is PreventT2 lifestyle change program?

PreventT2 is part of the National Diabetes Prevention Program, led by the Centers for Disease Control and Prevention (CDC). It is proven to help people with prediabetes prevent or delay development of type 2 diabetes. (Centers for Disease Control and Prevention, 2017)

With the PreventT2 program you get:

  • A CDC-approved curriculum as a part of the National Diabetes Prevention Program (National DPP).
  • The skills you need to lose weight, be more physically active, and manage stress and empower you to make a change for life.
  • A trained lifestyle coach to guide and encourage you.
  • Support from other participants with the same goals as you – and fun.
  • A year-long program weekly meetings for the first 4 months, then twice a month for the 5th and 6th month and once a month for the remaining 6 months.
  • To count as a goal for 2017 the participant would need to attend 14 of the 21 meetings between June 6th and December 5th. We want you to attend them all but are aware that things do come up and life happens. Then for 2018 come to the remaining 5 meetings (one a month).
  • Lose at least 5 percent of your current (first class weight) body weight, complete your weekly activity logs starting at week 3 and weekly food logs starting at the 4th week.
  • They are given information, tools, encouragement & guidance, develop a network with others in similar situations.
  • The program is at no cost to UVA/UPG employees as long as they remain in BeWell and an employee.

What is the Benefit of being Part of a PreventT2 Program?

As part of a PreventT2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes. These changes include losing a modest amount of weight, being more physically active, and managing stress.

Being part of a group provides support from other people who are facing similar challenges and trying to make the same changes you are. Together you can celebrate successes
and find ways to overcome obstacles. (Centers for Disease Control and Prevention, 2017)

What is the Time Commitment with the PreventT2 program?

Every Tuesday evening starting June 6th, 2017, meetings are held in our office at 675 Peter Jefferson Parkway Suite 430, from 5-6 pm. We ask you to arrive at least a few minutes early and as early as 4:30. It is a year-long program with weekly meetings for the first 4 months, then twice a month for the 5th and 6th month, then once a month for the remaining 6 months.

What is required as a part of the program?

  • Private weigh ins are required at the beginning of each session.
  • The goal of the program is to help you lose 5 to 7 percent of your current weight – that’s 10 to 14 pounds for a person weighing 200 pounds – by eating healthier and being more physically active. This will improve your health and cut your risk of developing type 2 diabetes.
  • Participation in the program over one year with weekly meetings for the first 4 months, then twice a month for the 5th and 6th month and once a month for the second 6 months to help maintain your healthy lifestyle changes.

How do I qualify to take the preventT2 program?

Be employed by UVA or UPG, a participant of BeWell and:

Be 18 years or older with a BMI >24 (>22 if Asian), AND No previous diagnosis of type 1 or type 2 diabetes, AND

Diagnosis of prediabetes by:

  • HbA1C 5.7% - 6.4%
  • OR Fasting Plasma Glucose 100-125 mg/dL
  • OR 2-hour Plasma Glucose  (after 75 gm glucose load): 140-199 mg/dL
  • OR Previous diagnosis of gestational diabetes (or having a baby weighing more than 9 lbs.) (may be self-reported)

 

Do I need to be referred by my doctor to attend the program?

BeWell does not require a referral from your doctor to attend the program. There are no insurance forms, payments or preauthorization needed.

What is Prediabetes?

Prediabetes is a blood glucose (sugar) level that is higher than normal but not high enough to be diagnosed as diabetes. One in three American adults has prediabetes, and most do not even know they have it. If you have prediabetes and do not lose weight or do moderate physical activity, you can develop type 2 diabetes within 5 years. (Centers for Disease Control and Prevention, 2017)

What is Type 2 Diabetes?

Type 2 diabetes is the most common form of diabetes. Most of the food we eat is turned into glucose, or sugar, for our bodies to use for energy. The pancreas, an organ that lies near the stomach, makes a hormone called insulin to help glucose get into the cells of our bodies. When you have type 2 diabetes, your body can’t use its own insulin as well as it should. This causes sugar to build up in your blood.

Type 2 diabetes is a serious condition. It can lead to health issues such as heart attack; stroke; blindness; kidney failure; or loss of toes, feet, or legs. (Centers for Disease Control and Prevention, 2017)

 

What other programs does the BeWell RN have?

BeWell—Population Health, RN Programs:

The BeWell Nurse believes all team members and their families should receive the best care our health system offers. The high utilizer and non-emergent emergency room programs are designed for the population health nurse to reach out to employees, and their family members that have illnesses, to assure they have the information needed to maintain their health. 

The BeWell nurse can:

  • help prevent unnecessary emergency room visits
  • promote health and wellbeing
  • provide referrals to the Behavioral Healthcare program
  • help patients sign up for MyChart
  • review and answer questions about discharge orders from the ED and hospital visits
  • help locate pharmacies, primary care providers and/or specialists
  • provide information regarding the cost savings of using urgent care offices and employee health for non-emergent needs
  • find a transportation resource if there are barriers to transportation
  • provide information about benefits UVA offers that may assist in saving money, such as HealthyRx
  • Find medical resources within the UVA Health System to assist participants in their healthcare.
